Prescription Benefits

- Prescription transfer form - Applies to new members that need to transfer prescriptions from a non-Kaiser Permanente pharmacy to a Kaiser Permanente pharmacy. Members can also call the pharmacy services department at 503-261-7900 or 1-888-572-7231 (toll free), Monday through Friday, 8 a.m. to 5:30 p.m. Please call as soon as possible to get this process started.
- Prescription Program Overview – Refill prescriptions online 24 hours a day. An email prescription reminder service is available as well as free home delivery service. Refill prescriptions by phone by calling 1-800-548-9809 (toll-free) Monday through Friday, 8 a.m. to 4 p.m. Please have your ID card and prescription information handy when you call.
