The CIS Benefits online enrollment system walks you through each step to successfully enroll you in the benefits offered by your employer. Before you go online you will need to print the New Hire Instructions for detailed step-by-step directions on how to complete your online enrollment.
Your employer should have provided you with a Username and Password to logon on. If not, don't worry. When you click on the link below, you will be given instructions on how to enter your default Username and Password. If you have problems logging on, please contact your HR/Benefits representative.
Are you enrolling dependents on your medical/dental coverage? Before you begin the online enrollment process, please read the Dependent Eligibility and Documentation Requirements to determine if your dependents are eligible to be enrolled. If documentation is required, please print and complete the form and return it to your HR/Benefits Representative when you have completed your online enrollment.
Below is a list of dependents that will require additional documentation in order to be enrolled on your medical/dental coverage. Complete forms must be turned in to your HR/Benefits representative.
Spouse
If you are adding a spouse to your medical/dental coverage, you must provide a copy of your marriage certificate or license to CIS before you enroll them in benefits.
Domestic Partner
If you are adding a domestic partner to your medical/dental coverage, a signed Certificate of Domestic Partnership or a Registered Certificate of Domestic Partnership provided by your residing county must be submitted to your HR/Benefits Representative. (Not all employers cover opposite sex domestic partners. Please contact your HR/Benefits representativet to verify eligibility).
Children up to the age of 26
Includes the natural child of the employee, spouse or domestic parter, adopted child/child placed in your home for the purpose of adoption, and a child for whom you or your spouse/domestic partner has a court order granting legal guardianship.
