Add/Delete Dependent

Do you need to add or delete a dependent from your coverage?  You can if you have experienced an IRS qualified family status change.  In this section you will find a list of qualifying events and, if required, documentation that you must provide to your HR/Benefits Representative.   Changes to your health coverage must be made within 31 days following the event.  


Add a Dependent

Before you add a dependent(s) to your medical/dental coverage, please read the Dependent Eligibility and Documentation Requirements to determine if your dependent(s) meet CIS' eligibility requirements.   You must have experienced one of the Qualified IRS Family Status Changes listed below:

        EVENT                                                           DOCUMENTATION REQUIRED

  1. Birth                                                        Must provide Social Security number upon receipt
  2. Marriage                                                Copy of your marriage certificate/license
  3. New Domestic Partner                       Must provide a signed Certificate of Domestic Partnership or provide a copy of the                                                                   Registered Certificate of Domestic Partnership filed with the county 
  4. Loss of other Group Coverage         Must provide a Certificate of Credible Coverage to show loss of coverage
  5. Legal Guardianship                             Must provide a signed Affidavit of Dependency and a copy of the court order
  6. Placement for Adoption                     Must provide a copy of signed court or other legal documentation.
  7. Court Order to Enroll Child                Copy of the court order or Qualified Medical Support Order
  8. Increase in work hours per week   If increase in hours results in a lower cost share, dependent(s) can be added

 


Delete a Dependent

Before you delete a dependent from your medical/dental coverage, you must have experienced one of the IRS Qualified Family  Status Changes listed below.  Only dependents losing coverage involuntarily are eligible for COBRA.  

      EVENT                                                           DOCUMENTATION REQUIRED

  1. Death                                                       Written notification only
  2. Divorce                                                    Copy of the signature pages of the Divorce Decree showing the date of dissolution
  3. Termination of  Domestic Partner   See below*  Termination of Domestic Partnership form required 
  4. Gain other Group Coverage               Written notification only
  5. Child no longer eligible                       Written notification for deleting child (marriage, other coverge, etc.)
  6. No longer required to provide coverage    Copy of Court order terminating requirement 
  7. Reduction in hours per week            If reduction in hours results in a higher cost share, dependent(s) can be deleted

 * In order to delete a domestic partner from your coverage mid-year, you must be paying the imputed tax value for this coverage.  If have not been paying the imputed tax, you are not allowed to delete a domestic partner mid-year.  The domestic partner can be deleted at open enrollment.
