Worksite Wellness Grants

CIS Benefits Staff and the Board of Trustees are pleased to again offer your entity the opportunity to apply for a 2009-10 Worksite Wellness Grant. Thank you for your support of the CIS Healthy Benefits program. Worksite Wellness Event Evaluation

Please complete this evaluation each time your workplace sponsors a worksite wellness event or program that used 2009-10 Healthy Benefits wellness grant funds.  Your ongoing feedback helps us evaluate CIS resources, plan for upcoming Healthy Benefits programs, and provide budget justification to the CIS Board of Trustees for program funding.  Please click on the link to submit your evaluation.   


Worksite Screenings

Region1 includes the Counties of, and Cities within those Counties of: Baker, Grant, Harney, Malheur, Morrow, Umatilla, Union, and Wallowa.  Screenings were held at these sites October 5-8.

Regions 2 – 6:  Matching Worksite Screening Grants

Member groups in counties other than those listed for Region 1 above, are eligible to apply for matching screening grants.  CIS will reimburse 50% of the screening cost up to a maximum of $22.50 per eligible participant for the below mentioned screening tests. 

  • Cholesterol - total value Resting blood pressure
  • HDL/LDL with ratio Resting heart rate
  • Triglycerides Glucose
  • Brief counseling session Height & weight (optional)

Flu Shot Clinics

If your group plans to host a worksite flu clinic this year, you must use an approved provider listed on the Regence  BCBS Provider list.  Regence will not reimburse vaccination cost administered by a non-Regence approved provider.  For more information about the flu shot program with Regence BCBS, click here.

Kaiser Permanente members must use a Kaiser Permanente provider.
