Weight Watchers Programs

Before you enroll in a program or pay for a voucher packet, you must complete an Interest Form to verify that you are eligible to receive reimbursement.  Fax your completed form to 503-375-7996.

Eligibility Requirements

You are NOT eligible for reimbursement if you have previously participated in two CIS-sponsored Weight Watchers series. However, you may still enroll and enjoy corporate pricing.

Weight Watchers, International Program Options

Enrollment Process

1. Verify your reimbursement eligibility by completing the CIS Lifestyle Program Interest Form and fax to CIS.  CIS will notify you if our records indicate you are not eligible for any financial program reimbursement.
2. Enroll by calling the Weight Watchers, Int’l Enrollment Center at: 1-866-442-1096.
3. Pay enrollment costs up-front using a personal credit card. You will receive your vouchers, Welcome Letter, and enrollment gift via mail.

•  10-week Local Meeting vouchers packet: Cost of $120.00

Enrollment Process

2. Enroll by calling the Weight Watchers of Southwestern Counties, Inc. Enrollment Center at: 1-800-651-6000, or 541-342-5386 (local number).
3. Pay enrollment costs up-front using a personal credit card. You will receive your vouchers and Welcome Letter via mail.

Reimbursement Maximum of $175.00 for the 2009-10 plan year: CIS will reimburse up to 70% of your fees for any one meeting series. If you enroll in additional series, you may apply for the difference between the reimbursement maximum of $175.00 and the amount you received for your first voucher series.

Reimbursement Requirements

1. Fulfill meeting attendance and all reimbursement criteria.   ATTENDANCE REQUIREMENT:  CIS policy allows ONE MISSED MEETING per meeting series.
2. You must begin your program no later than May 1, 2010.
3. Save the Proof of Receipt verification that is part of the voucher packet.  You will need this for the reimbursement process.
4. Vouchers have an expiration date. Please use your vouchers before the expiration date. Vouchers are the same as cash. THE 70% REIMBURSEMENT WILL NOT PAY FOR EXPIRED OR LOST VOUCHERS.
5. include a copy of your meeting attendance log with your reimbursement application.
6. Complete the Outcomes Survey.
7. Complete and submit the CIS Reimbursement Form along with the Outcomes Survey to CIS at the conclusion of your program
