Healthy Eating and Lifestyle Programs

The Healthy Benefits program is beginning year eight of program support for employees who are insured by a CIS sponsored medical plan and interested in making healthy lifestyle choices. Through the years, weight management has been the most sought after program. The science and research behind health, disease prevention, and weight management are pretty clear - good health is not about body weight alone. Healthy weight, and reducing risk for illness, are both about what we eat, how much we eat, and how physically active we are.
The good news is that our bodies respond immediately to improvements in eating and physical activity. It is never too late to make a lasting difference in personal health and eating habits in order to improve quality of life and reduce the risk for disease and disability. Healthy Benefits programs are committed to offering you opportunities to learn healthy habits. Our programs are based on the science of good health, balanced eating, physical activity, and sensible weight management.
Eligibility for Healthy Eating Programs & Incentive Reimbursement:
- Completion of the Health Status Questionnaire (HSQ)
- Active employee, retiree or spouse covered under a CIS sponsored medical plan
- Must complete a Interest Form to verify eligibility
- Must fulfill attendance and all program criteria
- The Healthy Benefits program will reimburse up to a maximum of $175.00 for the 2009-10 plan year for your continuous participation
- Must begin a program no later than May 3rd, 2010
Program Options:
You choose the program option that works best for you. The Corporate Weight Watchers Int’l. Agreement offers a 13-week or 18-week voucher meeting packet. Each option is bundled with a physical activity kit. Weight Watchers of Southwestern Counties, Inc. (Coos, Curry, Douglas, Jackson, Josephine, Klamath and Lane) are under a different Weight Watchers franchise. Program options and pricing are slightly different.
Dr. Linda Willis developed and instructs The Balanced Weigh (TBW) program. TBW teaches a reasonable costeffective way to eat and be physically active for health, weight management, and disease prevention. TBW includes an introductory class, a 7-week class series, and two personal consults with Dr. Willis.
To Get Started:
- Review the detailed program information regarding your program of choice.
- Complete the Healthy Eating and Lifestyle Program Interest Form and fax to CIS at 503-375-7996.
- Enroll directly in your chosen program following the steps outlined in the program details.
You are not eligible to receive the program subsidy if you previously participated in two CIS-sponsored Weight Watchers programs. If you have questions regarding your eligibility, please email: healthybenefits@cciservices.com.
