What is Healthy Benefits?
CIS Healthy Benefits is a valuable addition to your employer sponsored medical plan. The Healthy Benefits program is about YOU – and this year about YOUR FAMILY. Healthy Benefits provides personalized wellness programs for you and your spouse (including domestic partners) who are insured by a CIS medical plan.
The Health Status Questionnaire (HSQ) is a short 15-minute health survey that provides you with a snapshot report of your health. HSQ completion is required to participate in select program offerings (indicated by an *). To complete the HSQ, click on the link.
What’s Included with Healthy Benefits?
Health Coaching* – dedicated health coaches (health professionals) build on your strengths to help you make lifestyle changes that you have said you wanted to make.
Healthy Benefits for Spouses* – spouses & domestic partners may participate in all Healthy Benefits programs for which they may qualify. Completing the HSQ beginning in August is the first step.
Worksite Wellness Grants & Screenings* – help your employer group qualify for CIS worksite wellness grant funds by completing the HSQ. Screenings will be held throughout the state on a regional basis.
StayWell for Life Family Newsletter – mailed to employee homes on a quarterly basis. Look for information specific to CIS Benefits on the first & last pages.
All HSQ and program participation information is strictly confidential.
Healthy Benefits Programs
Tobacco Free Program* - offered through Healthy Future. Financial support for nicotine replacement and medication.
Healthy Eating & Lifestyle Programs* – a variety of community & online programs to help you reach your health goals.
New Healthy Benefits Program Partners
Health Future – based out of Portland, Health Future is providing the new HSQ, Health Coaching, and Tobacco Cessation program. Health Future works extensively with other public sector Oregon employer groups.
StayWell Custom Communications – hosts the health center on the Benefits Health Portal and publishes the StayWell family newsletter. StayWell is a national provider of consumer health information.
